Please make sure to review and complete the New Patient Registration Forms. You will also need your insurance card(s) along with a valid picture ID. Make sure to send copies of any relevant prior medical records and testing. An up to date list of medications should also be provided.This information should be emailed or faxed to us prior to any appointment so that we can have it on hand for your appointment.

Dr. Rubinov holds an academic appointment at the Icahn School of Medicine at Mount Sinai and is credentialed at multiple hospitals in the Mount Sinai Health System in both Brooklyn and Manhattan.

➢ Mandatory face coverings are required in the office and procedure area at all times.
➢ Non-urgent visits are being transitioned to telemedicine to limit patient and staff exposure.
➢ Patient escorts are encouraged to wait outside the facility rather than in waiting areas.
➢ Temperature checks are performed on all patients and staff members.
➢ All patients are screened for COVID-19 symptoms prior to and on arrival.
➢ Appropriate personal protective equipment (PPE) is worn by all staff members during procedures.
